Abstract

Obesity and diabetes mellitus are very prevalent metabolic disorders and cardiovascular risk factors that are common to coexist together. Each of which exhibits hazardous effects on left ventricular mass and left ventricular diastolic function leading to left ventricular hypertrophy and left ventricular diastolic dysfunction, which are known precursors of heart failure.
Abdominal obesity particularly due to excess of visceral fat is associated with increased risk of developing insulin resistance and concomitant cardiovascular risk factors. Hyperinsulinemia and the insulin-resistant state present in obesity and diabetes mellitus are the most possible factors leading to left ventricular hypertrophy and diastolic dysfunction as well.
The aim of this study was to evaluate the unfavorable effect of combined obesity and diabetes mellitus on left ventricular mass and left ventricular diastolic function by echocardiography in comparison to the effect of obesity alone.
To reach out this aim, sixty obese normotensive subjects characterized by abdominal obesity were examined by echocardiography and divided into two groups that were matched by age and sex; thirty obese diabetic patients as Group A and thirty obese non-diabetic subjects as Group B. Obese subjects were defined by BMI ≥30 Kg/m2 and waist circumference ≥ 94 cm for male and ≥ 80 cm for female. Obese subjects who had hypertension, ischemic heart disease, valvular or congenital heart diseases were excluded from this study.
