Abstract

Particular attention should be given to women during pregnancy IIld children during infancy and preschool age. The extension of family health services specially in rural communities from the health centre by an outreach program to focus on vulnerable groups would be lore effective. Home care services should not only contain nursing care services but also health education program deealing with mother and child health needs. The present study aimed to formulate and apply a family health . care program for families in Abbis 2 village inclusive of home nurs­ ing care and health education. Too intermediate targets include the identification of environ­ mental conditions of the home, health state of family members and the mother’s knowledge and practices about important aspects of health and family’ planning. The second target includes the planning and implementation of .the home care program for tweny families based upon their needs and testing their eff~ctiveness. The hypothesis of the study assumed that the home care program can help rural families to improve their living conditions and enable them to increase their capabilities in coping wi th their health needs. Also it assumed that the continuous follow up home visits will be useful in reaching and maintaining an optimal level of prog­ ram functioning. . The nursing process steps were used as a base for the home care prog­ ram acti vi ties. : The assessment ~hase: 1. Review of 11 terature related to the topic to be sure of the need of rural communi ties to outreach program. 2- Developing a questionnaire and its scoring system. 3- Preparation of the equipments and supplies needed for the assessment and implementation phase. 4- Selection of the samples and carry out a pilot study. 5- Home visits to the experimental and control families for assessment of the:i.r needs using observation, simple clinical examinations, anthropometric measurement and interview questionnaire. ~e result of the assessment phase revealed a real need for \ ~ outreach program including health education and nursing care services. This home care program needs to focus on ~tenatal care, natal, postnatal and neonatal care, family planning, care of infant and preschool children, geriatric care and home health condition to improve the’ family health, correct and improve the health information and increase the abilities in practicing healthy behaviour. . The planning phase: 1- The home care program was constructed with general and specific objectives derived from the results of the assessment phase. 2- The contents were selected after careful study of the expectant wives needs, interests, abilities, problems, their background education, knowledge and social characteristics. 3-Preparation of the selected contents using a simple scientific language. Differences were considered in selecting learning facilities. 4- Methods of teaching used were selected to suit the home situa- , tion. They were individual teaching, discussion and demonstra­ tion with the use of visual aids. 5- Construction of schedule for implementing the program to the families considering the duration of pregnancy assigning the priorities in starting the program. The visits were planned to be four visits during the antenatal period and six visits during the post natal period. Two visits were planned for follow up and six months to finish , the implementation period. 6- Evaluation was planned to be based on testing the knowledge and practice of the expectant wives before and after the program. Also to study the effect of the program on children, aged persons and outcome of pregnancy using pre and post prog­ ram questionnaires, outcome of pregnancy and children sheets and pictures to some parts of some houses before and after the program. 11/. The Implementation phase: The home care. program was implemented for every expectant wife in the experimental group according to the schedule of priori ty of visits and the nursing care plan of care. The program extended for six months started from December 1983 to end in May 1984 and covered 20 expectant wives 20 preschool children e.nd 10 aged persons. The results of the evaluation of the home care program: 1- The implementation of the home care program improved the know­ ledge of the expectant wives about the care needed during preg­ nmcy cycle, child rearing, family planning and care for aged persons. This was evident from the resultant significant changes after the program between the experimental and the control groups as well as between the experimental group before md after the program. 2- The practices of the expectant wives were improved in cleaning home practice, antenatal care practice, the practices of child care during first 2 years and up to six years old Be fore the program the percent of grade good in the different practices studied ranging 0-21 after the program implementation in the experiment group. While no significant differences were found in the control group before and after the program. 3- The program showed positive effect on the preschool children health condition as it helps in completing the vaccination of 6 children and four children got vaccination but not yet com­ pleted,they need more follow up. Also it improved most of the children weight percentiles. ~e program revealed good impact on reducing the post partum com­ plications among the experimental group to 10 while it was 90 in the control group. ~ Non of the newly born in the experimental group’ had any disorders compared to 77.8 of the control group who complained either from in!lamation of the umbilical cord or discharge in the eye, buttox rash and rash around the neck. 6. The impact of the program on aged persons was due to their responses with the health and nutritional instruction which resulted a positive change toward favourable personal hygiene behaviour and nutritional practice. This type of service is recommended to be implemented as a health services given by the health centre. It is to be avoided that any counteraction or resistance to be built against such a service either by the staff of the centre or the served popula­ lion. This service is to be integrated with the ongoing community health services and consequently it is going to be submitted to the con­ tinuous evaluation and remodel1ng to be dynamically fit with the social and familial changes. changed to 50-80
