Abstract

It is repeately documented that general hospitals ,are the most costly component of the health service system in almost all communities. The high expence is in both building and operating a hospital. Moreover, :n recent years, the costs of hospitalization have risen sharply beyond the expected limits. This rapid rise has encouraged increased interest in possible methods of decreasing costs without impairing the quality of me¬ dical care rendered. One important economic characteristic of hospital operation is la=ge proportion of fixed costs in relation to total costs. Thus, flxeJ h06D1¬ tal costs are estimated to be about two thirds or even Lllree quarLers ~r total costs. This high proportion makes the empty bed bery expensive ’0 maintain as the increment to cost of maintaining a bed which is occupi~d is small compared with the indirect and fixed costs involved in supporting this bed (when empty) with the entire complement of personnel, equipme~t and plant which modern medicine requires. Therefore, hospital costs r.~th capital and operating can be reduced if the number of un-occupied bedu can be reduced without reJuclng the demand for hospital carc. This could be achieved through careCul cstlmatioll oC Lilo nUlllber or beds required generally and departmental to cover the dcolnnd of certain popula~ion in Egypt who benefit medical insurance. Some important points were put under consideration as testing the effirien¬ ty of bed use generally and in each clinical department and assessing the I ~xtent of unnecessary use” of general hospital beds. Satisfying this study, retrospective and cross-sectional studies were o~e. . First class and ladies section showed more response in the A.L.O.S. than that of discharge rate. Th rest of the decartment showed the opposite. Also basic findings in tableco of analysis of Daily Census was that the coef icien t of variation as well as the days 0 overfilling in the consolidated sections were always lower than that of the individual deoartments.. It can be concluded, therefore that while some kind of departmentation is needed for a certain technical reasons, excess i ve depa rtmen ta ticn is undes irable, as it is Particularly difficult without excessive over loading and waiting lists. Thus for purpose of clanning on average occupancy of 75 ratio of 2 bed s per 1000 copulation will be enough to cope with the average demand for hospital care.
