Abstract Summary Our study subjects were 100 patients suffering from chronic suppurative otitis media (mucosal type) .their ages ranges from 9 _ 12 years and free from tonsils, adenoids, respiratory and urinary troubles. These patients took ear drops and classified into 4 groups aiming to reach the most effective ear drops to control infection with least possible ototoxic effect. 1st group : 2nd group: taking garamycin (gentamicin) ear drops. taking dexnpolyspcctmn (neomycin, polymyxin Band dexamethasone) ear drops taking decadron (dexamethasone) car drops . taking acetic acid 2”/0ear drops. 3 rdgroup: 4 th group: the dose of ear drops is 4 drops 4 times daily for 3 weeks in all these types. All patients did culture and sensetivity test before and after treatment and audiograms before and after treatment . Gentamicin and dexapolyspectran gave the best response as they eontroled infection in 100%of cases, while decadron controled infection in 76% of cases and acetic acid 2%controled infection in 72 % of cases. All these ear drops showed no audiological evidence of ototoxicity although ototoxicity of garamycin and dexapolyspectnm was proven in experimental animals in previous studies. 44 --- - -- -- -- ---’- - -- - ---- -- --- -._--.- -- - --- - ---- This may be due to the anatomic difference between human and animal round window which is exposed in animal while in man it lies at the apex of the deep narrow round window niche. However, this result will help us to use gararnycm and dexapolyspectran ear drops as a pre operative preparation and to control infection in whom surgery is contra indicated. while decadron and acetic acid can be used to control infection with other modalities of treatment |
