Abstract

Since Dallemagne et a1 first described the technique of laparoscopic Nissen fundoplication in 1991, extensive experience with antireflux surgery has been reported from many centres. The question of which technique offers the best outcome is still debatable. Only the outcome of randomized trials, if available, should be considered when this question is answered.With the objective of answering this question, 40 patients referred for antireflux surgery have been randomized into two groups; each underwent either a 360-degree laparoscopic fundoplication (Nissen), or partial posterior 270-degree fundoplication (Toupet). Both groups were followed up for 24 months for assessment of their subjective reflux symptoms and the appearance of new postoperative symptoms, through a standard questionnaire. They were also subjected to endoscopy,ambulatory pH studies and manometry 2 years after surgery, as objective means to assess the operative outcomes.
