Abstract

Flow obstruction may not be only a result of the stenotic area but a summation of obstructive effects of the reduction of valve area and the damage in the valve structure at valvular and subvalvular levels. The severity of the lesion in the mitral valve structure would influence mitral valve resistance calculated as mean transmitral pressure gradient/aortic flow ratio. Because valvular resistance appeared to be less dependent on conditions of measuement than valve area. The investigators (n4), suggested that valve resistance might be better indication of hemodynamic obstruction For two valves with similar areas and transvalvular flow. The valve with more severe morphological abnormalities would have a higher resistance than the other valve, and hence, worse functional class and lower exercise tolerance. To evaluate the value and determinants of mitral valve resistance and various methods of its measurements, fifty patients with symptomatic rheumatic mitral stenosis. (15 patients were candidate for balloon mitral valvuloplasty) were studied. All the patients had various degrees of pulmonary hypertension and NYHA functional class.
