Abstract
our study was done to compare the effects of NPPV and invasive PPV and also the effects of different tidal volumes on 50 patients with sever acute lung injury. Major risk factors were pneumonia, sepsis, massive blood transfusion, aspiration and pulmonary trauma. All patients received NIV in the form of BIPAP, 20 patients succeeded and 30 patients failed, those who failed were divided into 3 subgroups each of which contain 10 patients. NN group : Included 20 patients who succeeded NIV trial in the form of BIPAP through nasal mask with S/T (Spontaneous/ timed mode). InvasSe ttentSlat.ton group :- Included patients who failed NIV after 1 hour , they were divided into three subgroups :- Subgroup I : Included patients who received low Vt 4-6ml/kg with PEEP 10- 15 cm H2O. Subgroup I1 : Included patients who received moderate Vt 6-8 ml/kg with PEEP 10-15 cm H2O. Subgroup I11 : Included patients who received large Vt 8- 12 ml/kg with PEEP 10-15 cm H20.
