Abstract

The present study aimed to assess nursing intervention routinely provided for patients undergoing gastrointestinal endoscopy; to develop nursing guidelines for nurses attending gastrointestinal endoscopy; and to test the effect of these nursing guidelines on patients’ interaction and anxiety during endoscopy, as well as the outcome of the performed endoscopic procedure. This study was carried out at the Gastrointestinal Endoscopy Center in Mansoura University Hospital. The sample size for this study consisted of 400 endoscopy patients (200 patients before application of nursing intervention and 200 patients afterwards), in addition to all nurses working at the same setting. Five study tools were used: Patient’s interview questionnaire; Nurses’ interview questionnaire; Observation checklist for all nurses’ performance during the three phases; pre-procedure, procedure and post-procedure, as well as observations of the interaction between nurses and patients; the Hamilton’s Anxiety Scale for patients; and the Interaction Assessment Tool. In addition, the nursing guidelines were developed based on the analysis of collected data. Results indicated that, after the application of guidelines, the patients’ general preparation procedures became significantly more applied by nurses, also certain patients’ specific preparation for pre-endoscopic procedures increased significantly and less anxiety was experienced by endoscopy patients than before the application of guidelines. This study concluded that instructing the nursing guidelines for gastrointestinal endoscopy to nurses is associated with improved patient care before, during and after endoscopy, and well-prepared gastrointestinal endoscopy patients experience less anxiety associated with procedures of endoscopy than those who are less prepared. The study recommends the continuous education for gastrointestinal endoscopy nurses in addition to educating patients prior to performing gastrointestinal endoscopy. It is also advised that each patient would watch a videotape about his/her endoscopy with the nurse explaining each step and answering any related questions.
