Abstract

Cholestasis is defined as failure of normal amounts of bile to reach the duodenum. It may result from a wide variety of causes that may affect the liver or biliary tract. The aim of this study is to identify the various etiologic factors of infantile cholestasis in our community.
This study includes fifty patients suffering from cholestasis, 27 males and 23 females. Their ages range from 28— 270 days, with a mean age of 107.36 days. All had conjugated hyperbilirubinemia with a direct bilirubin fraction more than 20% of the total. All cases were subjected to careful history taking, general and abdominal examination, urine and stool analysis, complete blood count, standard liver function tests, serological tests for the various viral and metabolic profiles, ultrasonographic examination and percutaneous liver biopsy.
In this study; a positive reaction for toxoplasmosis was found in 10% of cases, antibodies specific for CMV were present in 8%, while 6% of cases showed the presence of HBsAg. Serological tests for rubella (MACRIA) and syphilis (TPHA) revealed negative results in all cases.
According to A1AT (El) phenotyping, 5 patients (l0%)were found to have A1ATD.In this study, percutaneous needle biopsy of the liver proved to be a safe, simple and reliable procedure. Definite pathological diagnosis could be achieved in all cases.
