Abstract

Obesity is considered as a major health problem and its complications are dangerous. So, its treatment is important, either medically or surgically. Gastric restrictive operation is used as a surgical line for treatment of obesity. Therefore this work proposed to study the complication of this operation on forty adult male and female rabbits, which were classified into two equal groups, control and experimental, the latter group was subjected to gastric restrictive operation. One and two weeks after the operation the body weight of each rabbit was recorded, then they were laparotomized under ether anaesthesia , and parts from the body and pyloric antrum regions of the stomach were taken an6 prepared for light and scanning electron microscopic studies. The animals subjected to the operation exhibited weight loss of about 100-150 gm per week in comparison with control group and the stomach decreased in size and adhered to the surrounding structures. Different types of gastritis were found, as an early postoperative complication after surgery, including erosive, haemorrhagic and reflux gastritis. Metaplastic changes in the surface epithelial cells and pyloric glands were also found, which represented a change of mucosal pattern from gastric to intestinal type, accompanied with conversion of some columnar epithelial Summary and Conchion cells into goblet cells,and decrease in the PAS positive material in the surface epithelial and mucous cells, due to decrease in mucus production in late stages after the operation. Examination of the luminal surface of the stomach at different periods after the operation by using scanning electron microscope showed scattered areas of mucosal necrosis and ulcers and in the non healed ulcers, there was proliferation of fibrous tissue. These changes associated with the presence of short bulbous microvilli, which projected from the free border of the surface mucous cells toward the lumen. Also , there was increase in mucus secretion in the close6 pa-; of the stomach body one week after the operation. This -GLJ>-S suggested to be partly as a protective mechanism to the in.uqi resulted from the operation, and partly due to retention of gastric secretions in the closed part. Then in late stages there was decrease in mucus secretion, also there were many rod and spherical shaped bacteria in the form of diplococci, supposed to be the aetiology of ulcerations and gastritis that was found after the operation.
