Abstract

After the introduction of cyclosporln as immunosuppresslve drug a remarkable increase In graft survival has been observed by most kidney transplantation centers (Morris, 1988b). Nevertheless, high cost and chronic nephrotlxicity stand as major challenge for long term use of this drug (Evans and Manniens, 1988; Holt’ et al., 1986).Ketoconazole, an imidazole derivative antifungal drug was reported to interact with cyclosporln through the cytochrome P-450 Microsomal enzymes in the liver, leading to inhibition of cyclosporin Metabolism and subsequently increasing its blood level (Rosano et al., 1988).The early reports of this drug interaction focused on Its nega¬tive impact on renal function (Lokeic et al., 1981; Daneshmend, 1982;Ferguson et al., 198? and Smith et al., 1983).The potential financial and safety benefits of concomitant therapy was recently reported in an unmatched series of transplant recipients (Schroeder et al., 1987; Butinan etHrl., 1989; First et al., 198941991). However, these studies lack the controlled group in their study design.So, the objective of this work is to study the long-term eva¬luation of concomitant ketoconazole and cyclosporln in a prospective randomized study.
