Abstract

The present study was conducted on 100 patients in the Psychiatric department Mansoura University hospital. These patients met DSMIV- TR criteria for bipolar I disorder. We subdivided them regarding absence or presence of psychotic features into three groups: Group I: Patients diagnosed as bipolar disorder without psychotic features (40 patients), Group II: Patients diagnosed as bipolar disorder with mood-congruent psychotic features (31patients) and Group III:. Patients diagnosed as bipolar disorder with mood-incongruent psychotic features (29patients). We compared between these three group in three main perspectives; Socio-demographic, clinical and cognitive data; family history of the proband regarding any mood or psychotic disorders in first or other degree relatives and molecular genetic study for 2 VNTR in dopamine transporter gene one previously studied 3 UTR and the other still novel one with little studies involving it (3 intron VNTR). This study demonstrated differences regarding age at onset, rate of episodes, previous hospitalization, previous ECT treatment and YMRS or HDRS total scores beside information from family history support the possibility that psychotic bipolar disorder particularly the mood-incongruent one- may represent a neurobiologically distinct subgroup of bipolar affective illness. And Attempts to draw a line of demarcation between schizophrenic and affective illnesses should be aborted and that bipolar I disorders with mood-incongruent psychotic symptoms are the epiphenomenon of the overlap, possibly genetic, of a ‘‘schizophrenic spectrum’’ and a ‘‘bipolar spectrum’’ and area between them as a kind of psychotic continuum. Despite that ,classifying bipolar disorder with incongruent psychotic features to be distinct diagnostic entity still unsettled
