Abstract

Gastric carcinoma occurs worldwide with a variable distribution from country to country and ranks the fourth in order of global cancer incidence Gastric adenocarcinoma represents > 90% of all gastric cancers. There are a variety of histologic classifications in use, the most frequent being the WHO and Lauren’s classifications. The WHO histologic classification is the simplest, easy to apply by different pathologists and the most reproducible. Lauren’s classification is used in the field of epidemiological study with great success. It delineates two major types of gastric carcinoma, the intestinal-type and the diffuse-type which differ in the biological behavior and etiology.The intestinal-type is more frequent and represents the dominant histological type in high risk areas suggesting environmental influences. The intestinal-type gastric carcinoma develops through a sequence of events whereby the normal gastric mucosa is affected by chronic gastritis followed by glandular atrophy, intestinal metaplasia then dysplasia prior to the onset of cancer. It is well established that H. Pylori is likely involved early in gastric neoplasia sequence. In contrast, the diffuse-type has not been related to precursor lesions and has a high association with familial occurrence. Worldwide, gastric carcinoma has a dismal prognosis. It ranks the second as a cause of cancer related deaths in the world. The 5-year survival rate in advanced stages is extremely poor. Prognostic parameters can be systematized in the well known TNM classification. Early gastric carcinoma without lymph node metastasis has a 5-year survival rate of over 90% and even in advanced cancer limited to the muscularis propria without lymph node metastasis, the prognosis is almost good.
