Abstract

The aim of the present work is to study some of the prothrombotic factors and endothelial function parameters after cardiac valve surgery, to find if there is a relation between the changes in these prothrombotic factors, endothelial function parameters and the early postoperative complications, also to find out if the changes in these parameters differ between open and closed valve surgery.The material of this study comprised 40 individual who were selected from the inpatients of the Cardiothoracic Surgery Department in Mansoura University Hospital as a test group and 10 healthy subjected of matched age as a control group. The test group divided into the followingGroup I: included 30 patient with cardiac valve disease undergo open heart surgery Group II: comprised 10 patients with MS undergo closed commisurotomy.All patients underwent through history taking and full clinical examination with special stress on cardiac related manifestations. Also, they underwent a group of investigation resting 12 lead ECG, echocardiography, plain x-ray chest, various biochemical tests: lipid profile, fasting and 2 hours post-prandial blood glucose, serumcreatinine, complete blood picture, erythrocyte sedimentation rate (ESR), liver function tests, E-selectin assay and von Willbrand factorOur study revealed the following results:Patients with valvular heart disease have some degree of endothelial dysfunction that is aggravated by cardiac surgery and this aggravation was more with open cardiac surgery than closed one.Age has a modifying effect on the changes in markers of EDs and the deterioration in endothelial function after open surgery were more in young patient than in older patients who may suffer from already present EDs.Gender difference did not alter the changes of EDs after open cardiac surgery Replacement in aortic position cause more deterioration in EDs. • Patient with larger LAD with associated more haemodynamic changes and shear stresses are liable
