Abstract

In our study we tried to improve hospital resources utilization, reduce health care cost and improve outcome among patients suffering from chest pain especially that they are not few and growing in the community with limited intensive care resources, by detection an ideal marker which is able to rule out any cardiac damage and identifies low and high risk patients. In this study we assessed the usefulness of measuring serum ACBT levels, CTNT, CKMB activity and HS-CRP in patients who were diagnosed either AMI or VA .The study was carried out in ICU of Zagazig University and included 60 subjects (42 males and 18 females) classified into: 16 patients with STEMI (group I), 13 patients with non STEMI (group II), 16 patients with UA (group III) and 15 healthy subjects (group IV ) . All patients underwent thorough history taking, clinical examination, serials ECGs and laboratory work up which included total CK activity, CKMB activity, CTNT, ACBT and HS-CRP that were measured twice, the 1st within 3 hours of chest pain and the 2nd after 24 hours of chest pain . from our study we found that for AMI patients, the 1st sample CKMB sensitivity was 20.7 % , specificity was 54.8 % , NPV was 57.45 % and PPV was 67.4 %. -{ CTNT sensitivity was 37.9 % , specificity 64.5 % , NPV 60 % and PPV 72.5 % .
