Abstract

The ultimate goal of treatment of destructive periodontitis is the regeneration of the periodontium on the surface of tooth roots that have been deprived both of their periodontal attachment apparatus, and have been exposed to plaque pathogens, as well as the oral environment. The formation of new attachment has been studied for many years; however, the keys to predictable success have not yet been acquired. Different grafting materials, guided tissue regeneration membranes and growth factors are currently used to achieve this goal. Bioactive glass is one of the promising synthetic alloplastic materials which showed osteoconductive and osteostimulatory effects in some recent studies. Bony glass has a narrower range of particle sizes of 300-355 µm which has been reported to be advantageous for guiding osteogenesis. 1- Clinical Part of the Study.2- In Vivo Animal Part of the Study.3- In Vitro PRP part of the Study.
Conclusions1- Bioactive glass can be used successfully for the treatment of intrabony periodontal defects rather than surgical debridement alone and it is compatible with human tissues and handled easily as a graft material.
2- Platelet-rich plasma as adjuvant to bioactive glass is considered one of the best treatment modalities of intrabony periodontal defects. The combination therapy gives better healing and more improvement in all clinical parameters.3- Platelet-rich plasma can be prepared in the dental office by withdrawal of small amount of blood to be used in minor surgical oral procedures and no need for the expensive equipments and complicated techniques.4- Purified bovine thrombin can be used to activate the platelets without doubt of inducing allergic reactions.5- The viability of platelets in platelet-rich plasma preparations is considered very crucial for the release of active growth factors and cytokines.
6- Platelet-rich plasma when mixed with bioactive glass gives a cohesive mass to the mixture and easily adheres to the defect site without floating by the blood emerging from the surgical site.
7- The influence of platelet-rich plasma on wound healing and regeneration is believed to be through the action of growth factors.
8- Recently, other molecules discovered in platelet-rich plasma like lipoxin A4 and RANTES have natural anti-inflammatory effect and could shift inflammation in the wound area toward the healing phase
