Abstract

Uptill now there is no direct method for determining the presence or absence of fetal pelvic disproportion in breech presentation, this problem if not discovered’ initially, it will be catastrophic for the mother as well as her breech fetus. This study was carried out on 45 cases attending the emergency and high risk units of obstetric and gynecology department of the Mansoura University Hospital having breech presentation and in labour. All cases were subjected to earful history taking, general, abdominal and local examinations. After that ultrasonic measurements of the head and abdominal circumferences as’ well as x-ray pelvic measurements for pelvic inlet and midpelvic circumferences were done . Fetal - pelvic index then was calculated from the formula: (HC-IC), . (HC-MC), (AC-IC), (AC-MC), from the sum of the two most positive circumference differences, the value of the fetal pelvic index is derived: Positive value identifies that the fetus is larger than the pelvis, (presence of disproportion) negative value means the fetus is smaller than the pelvis
