Abstract Since 1910, when Kienbock published his classic description of lunatomalacia. neither the etiology nor a reliable treatment for this condition has been established with certainity. There has been a consensus, based on roentgenographic and histolo¬gic data, that the changes seen in Kienbock’s disease are due to avascular necrosis of the lunate bone but, the actual cause leading to this vascular impairment has remained elusive. Many theories regarding the etiology of Kienbock’s disease have been postulated. Of them, the occurance of trauma to the wrist, and the presence of the so-called negative ulnar variance condition are the most important etiologic factors. The condition, typically, presents in the right wrist of an adult male, manual worker. The age dis¬tribution is wide, but most patients are between twenty and forty years of age. Diagnosis of the condi tion is . made strictly by radiographs. Four clinical and z oen’tgenogr-ap hf o stages have been des¬cribed. The classification has been useful in co¬ordinating a variety of recently recommended treat¬men t . methods. |
