Abstract

Aim of the work: To study the clinicopathological factors influencing the outcome of hepatocellular carcinoma with liver cirrhosis in patients undergoing hepatectomy Patients & methods: This study included 175 patients with liver cirrhosis underwent hepatic resection for hepatcellular carcinoma in Gastroenterology Center (GEC) Mansoura University in the period from october 1993 to June 2006 Results: The mean age of the patients was 54.8±9.2 years, male to female was 131 to 44 respectively. Right hypochondrial dull aching pain was the most common symptome. Ther was high inidence of hepatocellular carcinoma in rural areas with high prevelance of hepatitis C virus infection. Major hepatic resection was done for 65 patients, segmentectomy for 62 patients and localized resection for 48 patients. All patients had cirrhotic liver proved by hisopathological examination. Recurrence after resection was presented in 78 patients. The median survival was 24 months. The result of a univariate analysis using the Kaplan-Meier product limit method for the relations between clinical , pathological , and interventional variables for survival after resectional surgery were lymph node status, tumor differentiation, staging, tumor’s capsule, cut margin, vascular invasion and operative blood transfusion. Conclusion: Liver resection has been considered a reasonable first-line treatment for cirrhotic patients with HCC and preserved liver function.
