Abstract

Material & Method : The study was performed on 116 eyes of 66 patients, who were diagnosed as primary open angle glaucoma. All patients were examined Humphrey Field Analyzer (program 24­2), and HRT II. Tow classification were done into mild, moderate and advanced; one according to the visual field and the other according to the HRT stereometric parameters. Also the optic nerve head parameters including rim area, rim volume, cup shape measurement, height variation contour and mean retinal nerve fiber layer thickness measured by HRT II were analyzed for correlation with visual field indices mean deviation by Pearson<U+00B4>s correlation, and agreement between visual field and HRT examinations calculated by means of kappa coefficient. Results : The visual field groups include 32 eyes in mild, 40 eyes in moderate and 44 eyes in advanced group. But the HRT groups include 8 eyes in mild, 36 eyes in moderate and 72 eyes in advanced group. In this study there were statistically significant correlation between mean deviation and rim area, rim volume, cup shape measurement, mean retinal nerve fiber layer thickness. There were no statistically significant correlation between mean deviation and height variation contour. Measure the degree of agreement (Kappa) shows poor agreement (43 %) between visual field and HRT results in detecting glaucoma. Conclusion : The structural changes on the optic nerve precede functional changes in many if not most cases of glaucoma. Also the top four parameters (rim area, rim volume, cup shape measurement and mean retinal nerve fiber layer thickness) could be good indicator of the degree of glaucomatous ONH damage. There is poor agreement between visual field based and HRT diagnosis of glaucoma.
