Abstract

Introduction: While the neoplastic cells have been always in the center of interest in cancer research, recently more and more attention have been paid to the tumor stroma which is known to play an important role in tumor progression. In Egypt, colorectal cancer constitutes about 6.1 % of all malignant tumors .An inflammatory infiltrate within the tumor indicates a better prognosis.Matrix metalloproteinases (MMPs) are a group of enzymes thought to be responsible for both normal connective tissue matrix remodeling and accelerated breakdown associated with neoplasm development. Studying of the stromal changes may be very helpful in assessment of the progression of a given tumor. Aim of the work: 1. Study of the characters of the stroma of epithelial benign and malignant colorectal tumors as regards inflammatory cellular infiltrate and the expression of matrix metalloproteinases (MMPs). 2. Comparison between expression of MMPs in benign and malignant epithelial tumors. Materials and method: This retrospective study was conducted on 70 cases of colorectal epithelial tumors. Paraffin blocks were sectioned at 4-5?m, and stained by haematoxyline and eosin (H&E) stain and immunohistochemically for MMP-2 antibody. Results: The number of TILs was larger in earliest stages of colorectal cancers and decreased with the presence of metastasis. Degree of tissue eosinophilia was found to diminish in transition form less invasive to deeply invasive carcinomas. By using immunohistochimestry we have shown that MMP-2 is significantly expressed in colorectal carcinomas compared to adenomas and it was directly related to measures of tumor aggressiveness. Conclusion: In conclusion scoring of TILs and eosinophilic infiltrate is a simple and readily available method for routine pathological analysis and can provide additional prognostic information, defining more accurately subgroups of patients that may benefit from adjuvant therapies. MMP-2 is significantly expressed in colorectal carcinomas compared to adenomas. This can be of interest in using specific MMP inhibitors as newly developed therapeutic measures for colorectal carcinoma patients.
