Abstract

Our study was carried out on 160 patients, selected from the outpatient clinics and inpatient wards of the Nephrology department in the Urology and Nephrology Center, Mansoura University. Initially, the patients were assessed by thorough clinical and laboratory investigations. The patients were of seven groups, the first group included nephrotic syndrome patients and contained 20 patients, 11 males and 9 females with mean age J6.98 ± 13.22 years. The second group included chronic renal impairment patients and contained 20 patients, 15 males and 5 females with mean age 48.9 ± 15.36 years. The third group included healthy persons and contained 40 persons, 17 males and 23 females, with mean age 36.65 ± 11.44 years. The fourth group included kidney transplant recipients (early stable) and contains 20 patients, 13 males and 7 females with mean age 27.3 ± 9.21 years. The fifth group included kidney transplant recipients (late stable) and contained 20 patients, 18 males and 2 females with mean age 31.95 ± 11.65 years. The sixth group included early kidney transplant recipients (less than one year posttransplantation with impaired graft function) and contained 20 males patients, with mean age 35.85 ± 12. 45 years. The seventh group included kidney transplant recipients (late; more than one year, with impaired graft function) and contained 20 patients, 19 males and one female with mean age 33.25 ± 11. 54 years. Initially, all the patients were learned how to collect the 24hs urine samples and serum samples were obtained, then the 24hs urine collection creatinine clearance were estimated, and the creatinine clearance was estimated using the Cockcroft equation, then using our new equation which consider the surface area as a reliable factor. Finally, these results were compared to the isotopic clearance of these patients, which is considered the most accurate method for estimation of GFR (gold standard method). The results showed that the creatinine clearance evaluated by our equation has a better correlation coefficient with the isotope clearance (r= 0.97) in comparison to Cockcroft equation (r= 0.90) and that of 24hs urine collection creatinine clearance (r= 0.88).
