Abstract

Idiopathic scoliosis includes a wide age range of patients and may be subdivided into infantile (onset at less than 3 years of age), juvenile (onset between age 3 and 10), and adolescent (onset at the time of adolescence , after the age of 10 years) groups. While it is easy to arbitrarily divide idiopathic scoliosis into these three types, clinically the dividing line is not always clear and there is often overlap. The aetiology of idiopathic scoliosis is unknown; however, research into the vestibular function and genetic patterns of patients with idiopathic scoliosis appears to hold the most promise for unraveling the mystery. The various curve patterns seen most frequently in patients with idiopathic scoliosis have been well described. Any deviation from these patterns should trigger adiligent for a possible underlying cause of the curve. Infantile scoliosis may be divided into progressive and resolving types based on the RVAD and phase of the curve as described by Mehta. The progressive curve is identified by a RVAD greater than 20 degrees and shift from phase I to phase II. Progressive curves are best treated by serial casting under anesthesia is an attempt to correct the deformity. After correction is maintained for 6 months, casting may be discontinued, but the patient must be followed to skeletal maturity. Failure to control the curve by casting warrants operative intervention by either placement of a subcutaneous rod or spinal fusion. Juvenile idiopathic scoliosis occurs in patients from the age of 3 years to the onset of adolescent i.e. age of 10 year.
