Abstract

Forty five patients with benign biliary obstructions (common bile duct stone, residual stone or recurrent stone, stone impacted in the ampulla of water with stenosis) were divided into two groups. Group I 25 patients in whom sphincteroplasty was performed, table (2) shows the indications for this procedure, and for stones with stenosis of ampulla of water with relatively normal, common bile duct healthy wall and relatively riot dilated. Group II 20 patients in whom choledechoduodenostomy was practiced table (3) shows the indication of this procedure and essentially for thick walled markedly dilated common bile duct. Preoperative evaluation was assessed as regard Age, Sex, clinical presentation, hematological picture liver function test, kidney function test, radiological examination, preoperative preparations. In most instances, the best procedure was chosen by the circumstances of the particular case. The final decision for either a sphincteroplasty or cholechoduodenostomy in this work was made at surgery and several operative findings were helpful. The knowledge gained about the stones found in the common bile duct is even more useful.
