Abstract

Four hundred patients with intractable duodenal ulcer have undergone different surgical operations for treatment of their ulcer disease in Gastroenterology center between 1976 and 1990. Postoperatively, the cases were followed up and evaluated clinically and endoscopically. Symptoms of currence were found 139 cases, but recurrent ulcer was cumented in 63 of them. Furthermore, recurrence was cumented in 12 asymptomatic cases. Barium meal and laboratory studies were done for cases with documented recurrence. Upper endoscopy was the procedure of choice. The highest incidence of recurrence was recorded after highly selective vagotomy (32.3%), whereas combined vagotomy d antrectomy was associated with the lowest incidence of recurrence. Management of recurrent ulcer was primarily medical and e success rate of medical treatment was 63.8%. Reoperation was done for 14 cases. Four cases developed rerecurrence for ich medical treatment succeeded in 3 cases (75%) and surgery was done for the fourth case (25%).
