Abstract

Twenty patients underwent Endoscopic subfascial division of the perforating leg veins for treatment of chronic venous insufficiency in the vascular surgery unite, Mansoura University from December 1996; to January 1998. They were evaluated chinicaly and radiologically. The overall success rate was 90 % the incidence of failure was 10 % and occures in early cases in whom the tourniquet was not used. Early post operative evaluation has revealed excellent results. Although endoscopic subfascial division of the perforating veins is associated with littel or no morbidity. Potential limitations of the endoscopic subfascial divsion.of perforating veins as regard the technique include the inability to reach the first cockett perforator behind the malleolus in some patients. A further limitation is that the sever lipodermatosclerosis may resist optimal expansion of the subfascial space in the distal calf. The valvular competence of the perforators can not be assessed with endoscopy for this reason all subfascial perforating veins are interrupted during surgery.
