Abstract The aim of the present work ; is to evaluate the safety and effectiveness of large volume paracentesis in our patients with hepatosplenic schistosomiasis and tense ascites when isotonic saline is . infused during the procedure and comparing the effects of partial paracentesis with that of large volume paracentesis. Renin-angiotensin aldosterone system was studied during the procedure. Twenty cases of hepatosplenic schistosomiasis with tense ascites were studied. All these cases were subjected to thorough clinical examination and investigations including the weight, pulse, blood pressure, cognitive functions and daily urine output, haematocrite, S.albumin, prothrombin time, S.bilirubin, S.creatinine, plasma sodium, plasma potassium, plasma osmolality urine sodium and osmolality. Plasma renin activity and plasma aldosterone concentration were also assayed. Abdominal ultrasound was done to every patient. The patients were divided into two groups; group (A) submitted to large volume paracentesis. Group (B) submitted to partial paracentesis (2 liters). 1.V. isotonic saline was infused during the procedure in a total amount of 170 mlllitre of ascitic fluid removed to both groups. |
