Abstract

It is well established that IBS has recognizable symptoms of its own, and that it is very common in the population. Subsequently there is a great increase in the interest and study of this enigmatic condition, and is no longer a waste-basket diagnosis. Employing the Manning criteria, and careful examination to exclude symptoms and signs that might suggest structural disease, should permit an early positive diagnosis of IBS with a minimum of investigation. Psychological assessment, sigmoidoscopy and rectal biopsy, and anorectal manometric results can perform a finn confident diagnosis which is fundamental to successful management. It seems logic that clinical and manometric tools can directly diagnose the problem while the other investigations can help in elimination of the simulating conditions else. Psychological assessment for IBS patients confirms an association between the psychopathology and bowel habit in that anxiety is more common in patients with predominant diarrhoea, while depression is more among those with constipation. Most patients of IBS benefit from a caring approach, reassurance may be the most powerful therapeutic tool available.
