Abstract This study was carried out on 22 patients on maintenance hemodialysis, randomly selected from the dialysis unit of Mansoura Urology and Nephrology center. Among them, 20 patients were males and 2 patients were females aged 13 to 59 years. Initially, the patients were assessed by thorough clinical and laboratory investigations and their dialysis was assessed by urea kinetic modeling nomographic approach calculating KtiV. Their nutrition was assessed by measuring skin folds, midarm circumference laboratory parameters and nPCR. Also their neuromuscular functions were assessed by clinical examination, EMG, MNCV and fatigue test, their cardiac status was assessed by echocardiographic examination. Dialysis dose was reviewed to achieve a target KtiV (1.2 - 1.4) for patients on thrice weekly dialysis and 1.6-2 for patients on twice weekly dialysis. Also, their nutrition was reviewed to achieve nPCR 1.1-1.3 g/kg/day, caloric intake 30-40 K cal /kg/day through diet manipulation and support. The patients were assessed finally after 3 months on targeted, dialysis and nutrition by thorough clinical, laboratory, neurological (EMG-MNCV- fatigue test), cardiac (Echo) evaluation. Also their dialysis and nutrition were assessed by urea kinetic modeling (KW ¬nPCR). Our results showed that nPCR has increased significantly with increasing the dialysis dose (target KtlV), also serum albumin was significantly higher at the end of the study. The Kt/V had a beneficial effect on neuromuscular and cardiac functions. Also it had a positive impact on the patients well-being at the end of our study. |
