Abstract

One of the most exciting new’ilmovations, in otologic surgery in the past decade is facial nerve monitoring. It saves surgical time, and reduces the patients and surgeons anxiety levels. An increasing number of surgeons are using the technique in their surgical procedures. Now, the using electrified instnllnents allows rapid identification of the facial nerve and helps the surgeon to find the facial nerve in the dissection. Although it was first used in acoustic neuroma and parotid surgery, facial nerve monitoring will eventually be used for most, if not all, otological and neurotological cases. We recommend facial nerve· monitoring in every otological cases. The reasons to use facial nerve monitoring routinely are several. The surgeon never knows when it will be needed in a particular case, the operating room personnel becomes familiar with the equipment, and the surgeon learns how to interpret the sOlmds produced by the monitor and how to correlate them with surgical manipulations around the facial nerve. In all cases where the facial nerve may be encountered in the temporal bone, routinely use a mechanical pressure device attached to the cheek (WR-S8 stimulator/monitor), ”SACS”, and electrified micro-instruments and burs.
