Abstract

The understanding of any disease has been traditionally built upon the triad of diagnosis (What is it?); aetiology (What caused it?), and pathogenesis (How did it happen?). Current emphasis in medical education is on the latter two elements, cause and mechanism of disease, rather than on morphology. Yet the aetiology of most skin diseases is unknown. The pathogenesis is either understood fragmentarily or not at all. Hence, the application of the thesis of ”diagnosis of inflammatory skin diseases by histopathological patterns” postulated by Ackerman (1978b) was inspired in this study. Different skin diseases of heterogenous aetiology and pathogenesis were grouped and studied clinicopathologically under the term ” angiocenteric dermatitis” in an attempt at better understanding of this group of diseases and differential diagnostic approach. These diseases revealed common stem of normal overlying epidermis when appreciated by scanning magnification. The exceptions were those of necrosis and blister formation due to underlying ischemia as shown in some cases of cutaneous necrotizing vasculitis (CNV). Biopsies of lesions from thirty-five urticaria patients led to the establishment of three histopathological subgrougs of urticaria.
