Abstract

This study has been planned to evaluate the effect of mivacurium patients with periportal hepatic fibrosis. Twenty patients were included in this study, divided equally into two groups, control healthy group and hepatic group submitted to operations to relieve portal
hypertension.
All patients were subjected to general anaesthesis : induced with thiopenton sodium (7 mg/kg) followed by mivacurium (0.15 mg/kg) to facilitate endotracheal intubation and maintained by NzO : 02, and halothane 0.2 % . Datex relaxography was used for stimulation of ulnar nerve, as well as for sensing the integrated electromyographic response. The device displayed T 1 and TOF ratios. The onset of action of mivacurium is measured from administration of the drug till suppression of T 1 ratio to 45 % and TOF ratio to 75 %, where endotracheal intubution was exc~llently performed. The duration of action also measured from the time of administration till recovery of T J ratio by 35 % and TOF ratio by 25 %.
This study revealed that the onset was delayed and nearly doubled in hepatic group in comparison with control healthy group. The duration of action of mivacurium was prolonged in patients with hepatic insult more than that measured with healthy group.
