Abstract

Carcinoma of the prostate is the second leading cause of cancer related deaths. Detection of cancer prostate in its preinvasive stages may offer a promise for studies of chemoprevention. Purpose: This study aimed at determination of the incidence of different premalignant lesions in the prostate gland and the association of these lesions with prostate adenocarcinoma. Materials and method: The present study was performed on 410 prostate biopsy specimens. They were collected from Mansoura Urology and Nephrology center, Mansoura university in a retrograde manner. Three types of biopsy specimens were encountered: needle, Trans­urethral resection and total prostatectomy specimens. The cut slides were used for staining with routine H&E stain as well as immunohistochemistry for High Molecular Weight Cytokeratin (HMWCK) in problematic cases. Results: The mean age of patients was 59.2 years. Foci of PIN were encountered in 130 cases. Atypical adenomatous hyperplasia was found in 12 cases. Four needle biopsy specimens contained foci of atypical small acinar proliferation. Using HMWCK, basal cell layer disruption was encountered in 45% of low grade PIN and 61 % of high grade PIN cases. Meanwhile, all AAH cases showed attenuated interrupted basal cell layer. Conclusion: High grade PIN is the most likely precursor lesion for prostate adenocarcinoma.
