Abstract

Introduction: Spontaneous bacterial peritonitis (SBP) is defined as bacterial infections that occur in patients with cirrhosis and ascites without any significant intraperitoneal infection. Aim of work : The aim of this study is to determine the role of ascitic and serum Amyloid A level as an early novel marker to diagnose spontaneous bacterial peritonitis possible. Also, to compare it to other markers as C-reactive protein (CRP) and leukocytic esterase . Material and methods: sixty patients with cirrhotic liver disease with ascites fulfilled inclusion and criteria are involved in the study.thirty of the patients are cirrhotic patients complicated with spontaneous bacterial peritonitis and the other 30 patients are non-complicated ascites patients as a control group .All participants of the study were subjected to full history taking, clinical examination and routine investigations. Serum and ascitic Amyloid A level were measured by ELISA method. Results :level of serum and ascitic amyloid A are significantly elevated in cirrhotic patients complicated with spontaneous bacterial peritonitis compared to non-infected ascitic patients. Summary:Spontaneous bacterial peritonitis (SBP) is a dangerous bacterial infections that occur in patients with cirrhosis and ascites .Diagnosis of spontaneous bacterial peritonitis (SBP) is dependent mainly on count of ascitic fluid polymorphonuclear leukocytes Serum amyloid A (SAA) is also an acute phase marker that responds rapidly to inflammation.The present study concluded that level of serum amyloid A is significantly elevated in SBP patients compared to non-infected ascitic patients.
