Abstract

The aim of the study was carried out to evaluate the clinical effect of NSF in comparison to SDF in management of deep carious lesions. Thirty-six patients were included in the study and randomly allocated into three groups, AgNPs group (A1): patients with deep carious teeth received conservative caries management (selective caries removal to soft dentin) and then the (NSF) was applied to remaining discolored dentin for 2 minutes, then the teeth were restored with resin composite restorative material. SDF group (A2): patients with deep carious teeth received conservative caries management (selective caries removal to soft dentin) and then SDF was applied over the remaining discolored dentin, then the teeth were restored with resin composite restorative material. Control group (A3): patients with deep carious teeth received conservative caries management (selective caries removal to soft dentin) and then teeth were restored with resin composite restorative material. For all groups, teeth were isolated by rubber dam. Access through the cavitated enamel and outline form were obtained, selective caries removal to soft dentine strategy was performed using hardness criteria. Total caries removal to hard dentine was used for the peripheral walls. For the pulpal floor soft dentine was left to avoid pulpal exposure. SDF or NSF were applied and all teeth were restored with composite resin restorative. All patients received postoperative instructions and oral hygiene guidelines that should be followed during the follow-up period. Patients were recalled after 3 and 6 months to follow-up the pulp vitality.
Results:
1) For SDF group, at 3 months all teeth were successful. After 6 months, a single case failed.
2) Regarding the NSF success rate in the current study all cases were successful after 3 and 6 months.
Summary & conclusions

3) For the control group, at 3 months all teeth were successful while at 6 months 2 cases failed.
4) No statistically significant difference in the success rate was found between different materials.
Under the limits of the present study, it could be concluded that:
1) Silver nano particles could be used efficaciously to treat permanent teeth with deep carious lesions
2) Application of 5% Nanosilver to fluoride varnish has same clinical efficacy as SDF in preventing the progression of dentinal caries.
3) Partial caries removal strategy is an efficient technique for management of the deep carious lesions.
Recommendations:
1) Further clinical randomized trials with longer follow up period are needed to confirm the results of this study.
2) More studies are needed with AgNPs edge and rod shapes, and different sizes and concentrations.
3) For simplicity of use, a further development should be tried regarding incorporation of AgNPs in the resin composite restorative.
