Abstract

Hypotension is a common side effect after spinal anesthesia in parturient. The incidence ranges from 50% to 80%. It develops in proportion to the extent of sympathetic blockade and sensory block height.
So this study aim was prediction of hypotension after spinal anethesia in women undergoing elective cesarean sections.
This prospective observational study was carried out at the operating theaters in Suez Canal University hospital in Ismailia on full term pregnant women aged between 20 and 40 years undergoing elective Cesarean Section under Spinal Anesthesia. High-risk pregnancy (placenta previa, abruptio placentae, eclampsia or preeclampsia), pregnant with cardiovascular comorbidities, contraindication to central neuraxial block and who had gestational diabetes mellitus were excluded from the study.
The incidence of hypotension defined by standard criteria (MAP < 65 mmHg or SBP < 90 mmHg) after spinal anesthesia, 76\130 women (58.5%) developed hypotension. Age, residency, parity and gravidity showed insignificant differences between women who developed hypotension and who didn`t as p>0.05.
Anthropometric measures of the study population. The mean BMI was 28.5 ± 5.56 kg/m2. The mean intra-abdominal pressure was 14.43 ± 1.65 while abdominal circumference was 104.29 ± 7.32. Also women who developed hypotension had significantly higher height, weight, BMI, IAP, AC, IAP\AC ratio and IAP\BMI ratio than women who didn`t as p<0.05.
Baseline HR, SBP, DBP and MAP showed insignificant differences between women who developed hypotension and who didn`t as p>0.05.
The outcome finding among the study population. The mean fluid used was1343.31± 483.34 ml.
