Abstract

Approximately one-half of the ambulatory primary care for adults older than 65 years is provided by family physicians, and approximately 22 percent of visits to family physicians are from older adults. It is estimated that older adults will comprise at least 30 percent of patients in typical family medicine outpatient practices, 60 percent in hospital practices, and 95 percent in nursing home and home care practices. The geriatric assessment is designed to evaluate an older person’s functional ability, physical health, cognition and mental health, and socio-environmental circumstances. It includes an extensive review of prescription and over-the-counter drugs, vitamins, and herbal products, as well as a review of immunization status. This assessment aids in the diagnosis of medical conditions; development of treatment and follow-up plans; coordination of management of care; and evaluation of long-term care needs and optimal placement.
Geriatrics or geriatric medicine is a specialty that focuses on health care of elderly people. It aims to promote health by preventing and treating diseases and disabilities in older adults. There is no set age at which patients may be under the care of a geriatrician or geriatric physician, a physician who specializes in the care of elderly people. Rather, this decision is determined by the individual patient’s needs, and the availability of a specialist. It is important to note the difference between geriatrics, the care of aged people, and gerontology, The management of elderly care require long-term care and assessment of their bio psychosocial needs in the short, medium and long term, in order to prevent adverse health outcomes: falls, insufficient care, disability, institutionalization, recurrenthospitalizations, and death.
