Abstract

Umbilical vein blood flow (UVBF) is increased while impedance to blood flow in the uterine and umbilical arteries is reduced in pregnancies complicated by fetal macrosomia and diabetes. Aim and objectives: To assess relationship between UVBF and DM in pregnancies with fetal macrosomia by comparing UVBF in diabetic and non-diabetic patients with fetal macrosomia. Subjects and methods: An observational cross sectional study conducted in Suez canal university hospitals at obstetrics and gynecology outpatient clinic on pregnant women with singleton macrosomic fetuses between 36-37weeks who were subdivided into 2 groups : group (1): pregnant women with Pre-gestational diabetes and have macrosomic fetus, group (2): pregnant non-diabetic women who have macrosomic fetus .both groups underwent umbilical vein Doppler assessment to asses umbilical vein blood flow. Results: Mean umbilical vein diameter is significantly higher in diabetic group compared to non-diabetic (1±(0.09) cm and 0.86±(0.04)cm ; P < 0.001) respectively. also umbilical vein blood flow was significantly higher in diabetics compared to non-diabetics ( 345.5±68.26ml/min and 255±24.8 ml/min; P < 0.001) respectively. Additionally, there was no statistically significant difference Between both groups regarding time average maximum velocity as it range between(11.8-19.4) cm/sec in diabetic group and between (12.1-15.8)cm/sec in non-diabetic group. Conclusion: This study found that umbilical vein blood flow increases in macrosomic pregnancy with pre gestational diabetes by about 35.5%comparedto nondiabetic macrosomics . It was as a result of increase in umbilical vein diameter and flow velocity inside umbilical vein.
