Abstract

Mechanical ventilator is a lifesaving adjunct to the treatment of critically ill children who are unable to maintain adequate gas exchange. Mechanical ventilation is also indicated when noninvasive management modalities fail to support oxygenation and ventilation adequately in cases of acute lung injury (acute respiratory failure, trauma) apnea with respiratory arrest that requiring intubation, acute or chronic respiratory acidosis and Hypoxemia. Mechanical ventilation is one of the most common procedures performed in PICU. The aim of the study is to assess the relationship between provided nursing care for children on mechanical ventilation and their clinical outcomes. The study was conducted in pediatric intensive care unit at Alexandria children hospital(AUCH) at Elshatbey . The subjects comprised of 30 nurses who worked in PICU and 50 children on mechanical ventilation.Three tools were used to collect the necessary data. The first tool wascharacteristics of nurses and children structured interviewed schedule which developed by the researcher to assess the characteristic of children and nurses.The second tool is provided nursing care for children on mechanical ventilation observational checklist it include two parts ,partA is nursing assessment that was adapted and modified from the American heart association and Americanacademy of pediatrics andpart B was included provided nursing intervention ,which include six checklists for suctioning ,positioning ,chest physiotherapy, skin care ,eye care and oral care ,Tool three was clinical outcomes of mechanically ventilated children observational checklist.
Content validity of tools were tested by five experts in the pediatric nursing field. Reliability of tools were ascertained using Cronbach’s alpha Coefficient. Reliability for tools were 0.92.
Every nurse was interviewed individually to obtain his or her characteristics. characteristics of children were obtained from child’s medical record. Clinical outcomes of children were assessed three times. The data was collected within a period of 12 months starting from July 2019 to the end of June 2020.”
The main findings of this study showed the following:
• Approximately one third of studied nurses aged from 25-30 years (33.3%).
• About 60% of nurses had bachelor degree.
• Less than half of studied nurses 40% were less than 5 years of experiences.
• In addition to 80% ofstudied nurses were attended training courses on mechanical ventilation.
• Also 80% ofstudied children were aged less than 12 monthes,54% of them were females.
• 48% of studied children were admitted to PICU diagnosed as respiratory failure.
• 80% of studied children did not have previous admission to PICU
• Allstudied nurses had satisfactory level of performance.
• There were no statistically significant differences regarding total mean scores of nurses’ performance and their characteristics.
• Incidence of ventilator associated pneumonia is 100% absent on admission as compared to 70% after 3 days from admission and 66%after 5 days from admission.
• About 62% of studied children had low risk of pressure ulcers
• 100% of children hadn’t eye infection
• 1% of children had oral fungal infection
• There was no statistically significant relation between clinical outcomesand the total mean score of nurse care performance .
