Abstract

Small fiber neuropathy (SFN), manifesting by sensory and autonomic dysfunction, has many potential causes including diabetes in about one third of cases, but 30-50% of cases are idiopathic. While large nerve fiber function can be tested with routine nerve conduction studies, SFN often requires more specialized tests like the quite invasive skin biopsy. Research suggests somatic sensory and autonomic small nerve fibers may be separately involved. More research is needed to relate SFN symptoms and signs to objective test findings, determine factors impacting severity and quality of life, and screen earlier in diabetes using reliable, easy and readily available biomarkers, as diagnosis unfortunately in many circumstances becomes established late.
