Abstract

Summary
Severe acute respiratory distress coronavirus 2 is the causative agent of the newly identified coronavirus disease 2019 (COVID-19) (SARS CoV-2). Infections caused by coronavirus may produce a wide spectrum of symptoms, from moderate to life-threatening conditions including viral pneumonia accompanying systemic damage. Pregnant women may be more susceptible to developing respiratory infection problems, which may increase the risk of both maternal and fetal morbidity and mortality due to the physiological and immunologic changes that occur during pregnancy.
Aim of the study to evaluate the effect of implementing motivational guideline for pregnant women to comply with WHO recommended preventive measures standard during COVID 19 pandemic. This was achieved through:
1- Assessment of pregnant women’s knowledge and compliance with WHO preventive measures standard during COVID 19 pandemic before motivational guideline.
2- Develop and implement a motivational guideline for pregnant women to comply with WHO preventive measures standard during COVID 19 pandemic.
3- Evaluate the effect of implementing the motivational guideline for pregnant women on women’s knowledge and compliance with WHO preventive measures standard during COVID 19 pandemic.
Research Hypothesis:
Implementation of the motivational guideline will improve pregnant women comply with WHO preventive measures standard during COVID 19 pandemic. A quasi-experimental study design was used to achieve the aim of the current study.
Settings: The current study was conducted at Ante natal outpatient clinic in Ain Shams Maternity University Hospital.
Sample: A simple random sample was used to recruit 68 primigravida women that calculated according to this statistical formula n = Z21- /2p (1-p)/d2. Z: statistic for a level of confidence. (For the level of confidence of 95%, which is conventional, Z value is 1.96). P: expected prevalence or proportion. (P is considered 0.5) d: precision. (d is considered 0.05 to produce good precision and smaller error of estimate) Pregnant women was recruit based on the following inclusion criteria; able to read, at first trimester, free from any medical or obstetric complications, and wasing to participate in the study.

Three tools of data collection was used to carry out the current study namely; structured interview questionnaire, pregnant woman’s comply with WHO preventive measures standard during COVID 19 pandemic self reported practices, and pregnant woman’s motivation.
Aim of Result
To fulfill the aim of the intervention, the collected data were analyzed, tabulated and presented in the following order:
Table (2) Shows that (35.3%) of studied women were in the age group 20-25 years with mean age was 29.37±6.21 years. (66.2%) had intermediate education. Additionally (67.6%) were urban and (100%) had a not worker, besides (55.9%) were enough income, while (58.8%) of the women under study were number of family members from 2 to 3 and (100%) of the studied patients were owners of housing.
Table (2) Explains that, (61.8%) of the studied women were number of pregnancies 2 to 3; while, (76.5%) of them were parity 1-2. Concerning mode of delivery (61.8%) of studied women had vaginal delivery; and 64.7% of them hadn’t abortion. As regard gestational age 51.5% of the studied women their GA were 8-10 wks, also all of them had no problems in the current pregnancy & (82.4%) of them had regular follow-up during pregnancy.
Table (3) Indicated that, there was an improvement on pregnant women’s knowledge about Corona virus post implementation of motivational guideline with highly statistically significant difference at (P<0.001) between pre and post implementation.
Before guideline implementation, nearly three quarters and more than three quarters (73.5%, 80.9% and 83.8%) of pregnant women had incorrect knowledge about methods of coronavirus treatment for pregnant women, incubation period for the virus & availability of coronavirus vaccine for pregnant woman respectively. While after guideline implementation, the majority (94.1%, 91.2% and 86.8%) of pregnant women had correct knowledge about methods of coronavirus treatment for pregnant women, incubation period for the virus and availability of coronavirus vaccine for pregnant woman respectively.
Table (4) Display that, 61.8% of pregnant women had unsatisfactory level of total knowledge regarding corona virus before implementing a motivational guideline. While after implementing a motivational guideline 92.6% of pregnant women had satisfactory knowledge.

Table (5) Indicated that, there was a marked improvement in pregnant women comply with the World Health Organization standard for preventive measures during the Corona virus with highly statistically significant difference at (P<0.001) between pre and post implementation.
Before guideline implementation, less than half (41.2%, 44.1% and 41.2%) of pregnant women had never practice about the hand washing, using personal protective measures during COVID-19 respectively. While after guideline implementation, the majority and nearly two third (97.1%, 86.8% and 64.7%) of pregnant women had always about the hand washing, using personal protective measures during COVID-19 respectively.
Table (6) Presented that 89.7% of pregnant women had unsatisfactory level of total practice regarding comply with WHO preventive measures standard during COVID 19 pandemic pre guideline implementation. While after guideline implementation, 75% of pregnant women had satisfactory level of total practice regarding comply with WHO preventive measures standard during COVID 19 pandemic.

Table (7) Reveals that there is highly statistical significant improvement on all subscales of pregnant woman’s motivation “perceived vulnerability, perceived severity, perceived reward of maladaptive behaviors, perceived self-efficacy, perceived response cost, fear, protection motivation, and behavior after implementation of motivational guidelines (p= 0.001**)”.
Table (8) Point out that, there were highly statistically significant relation between level of total knowledge post implementation regarding Corona Virus and their education level, number of family members at (p-value <0.001). Also, there were statistically significant relation between total level of knowledge and their household income at (P<0.05). While, there were no statistically significant relations between total level of pregnant women’s knowledge regarding Corona Virus and socio-demographic data pre implementation of guideline.
Table (9) Display that, there were highly statistically significant relation between total level of pregnant women’s self-reported practices and their education level, household income and number of family members at (p-value <0.001) post implementation of guideline. While, there is no statistically significant relation between total level of pregnant women’s self-reported practice and socio-demographic data at p-value (p>0.05) pre implementation of guideline.
Table (10) Point out that there were statistically significant positive correlations between total score of knowledge regarding Corona virus, total score of pregnant woman’s comply with WHO preventive measures standard during COVID 19 pandemic self-reported practices, and total score of pregnant woman’s motivation.
Conclusion
Based on the findings of the current study, it was concluded that study hypothesis was supported and motivational guideline and esucational sessions had a positive effect on the improvement of pregnant womens knowledge, comply with WHO preventive measures standard and motivation regarding COVIED-19.
Moreover, there were statistically significant positive correlation between total score of knowledge regarding Corona virus, total score of Pregnant woman’s comply with WHO preventive measures standard during COVID 19 pandemic self-reported practices, and pregnant woman total motivation score.

Recommendations
Based on the finding of the present study the following are recommended: Implementation of the motivational guideline at different antenatal clinics to improve pregnant women complies with WHO preventive measures standard during COVID 19 pandemic Further study: Study the determinant of pregnant womans’ compliance to WHO recommended preventive measures standars during Covid 19
