Abstract

Cannabis is the most used drug world‐wide, with an estimated 188 million recreational users in 2017 (approximately 3.8% of the world population). Paralleling population increases, the number of cannabis users has increased 16% between 2006 and 2016. Cannabis use disorder (CUD) is defined as problematic cannabis use leading to clinically significant impairments or distress. Motivational Interviewing has been widely implemented to help people change their behavior. Varenicline is a promising therapeutic candidate for CUD because it has activity at α7 nicotinic acetylcholine receptors, to which tetrahydrocannabinol, the psychoactive agent in cannabis.
We recruited 90 cannabis users and were randomized over three arms, Varenicline arm, Motivational arm, and Mixed arm, comparison of baseline characteristics showed no statistically significant difference between study groups in terms of age, gender, body mass index, marital status, education, residence, 1st time of substance intake and Beck’s depression score. In the current study, there was no significant difference between study groups in the patient’s compliance with p value 0.999. At the end of the study, Beck’s score lowest in mixed, followed by Varenicline and highest in Motivational, the differences were significant between the groups, indicating that mixed group had the lowest depression rates with p value 0.0001. same difference was noted in Beck’s anxiety score as post interventions score was lowest in mixed, followed by Varenicline and highest in Motivational, the differences were significant between the groups, indicating that anxiety was the lowest in mixed group.

To our knowledge this is the first study to compare varenicline, versus motivational interview versus both, however some studies has reported the efficiency of each modality alone in management of cannabis use disorders.
We finally concluded that combined varenicline and motivational interview approach showed the highest efficiency in terms of low Cannabis withdrawal scale, beck’s anxiety score and depression score compared to varenicline or motivational interview alone. Compliance to treatment was high and comparable in all treatment groups.
