Abstract

This was a prospective clinical trial that was conducted at 2 different IVF centers (Elneel IVF center at kornish Elneel st. Minia Egypt. and EL Nada IVF center, portsaeed st. Bany Sweef, Egypt.). In the period between 1st March 2021 – 1st March 2023 aimed to evaluate the effect of using TPEN (N,N,N′,N′-tetrakis(2-pyridylmethyl) ethane-1,2-diamine) ,a Zinc chelator on the immature human oocyte. for in vitro activation and maturation of oocyte.
After ethical committee approval all the patients were counseled and consented for the procedure
The studied cases were divided into 2 groups
study group 40 patients
Control group 25 patients
Both groups submitted to the same induction protocole , after oocyte retrival the case groups’ oocytes were treated with the TPEN then observed for the changes in oocyte maturation and entering the M2 stage
The resulted data were processed and presented as previously mentioned which showed that Zn+2 chelation and specific chelator agent such as TPEN could be effectively used as a new modality of IVM by incubating the immature human oocytes with this agent for specific time and concentrations.
Results of the current study showed that there is a statistical difference between the results of the 2 groups , moreover the results illustrated that the mean incubation time of TPEN was 36.9 (±6.69 SD) with range (25-45) minutes, the mean proper concentration of TPEN was 209.75 (±38.13 SD) with range (150-250) these numbers are where the most accepted results were met regarding the oocyte activation and entering the MII phase
Incubation time of TPEN above 42.5 was associated with negative fertilization with sensitivity 86.7%, specificity 100%, positive predictive value 100%, negative predictive value 92.6% and accuracy 95%
Concentration of TPEN above 230 was associated with negative fertilization with sensitivity 86.7%, specificity 88%, positive predictive value 81.3%, negative predictive value 91.7% and accuracy 87.5% The present investigation has determined that TPEN, a chelator of Zn2+, exhibits the capacity to enhance oocyte maturation and developmental capability. The utilization of Zn2+ chelator-mediated oocyte activation strategies presents a distinctive means of activating oocytes, which can effectively enhance the existing techniques of Ca2+-based activation. This synergistic approach has promise for improving the overall efficacy of oocyte activation.
