Abstract

This was a randomized controlled clinical trial that was conducted on 20 patients of tongue
SCC and required a hemi glossectomy and reconstruction by an ALT flap. Patients were recruited
into 2 groups a study group and a control group 10 patients in each. Patients in the study group
had their ALT flap virtually planned preoperatively while those in the control group had their ALT
flap design based on the surgeon’s experience. Both groups have been evaluated clinically and
radiographically to determine volume sufficiency of ALT flap in relation to the defect size and to
assess for function (including speech and swallowing.). Results of both groups were statistically
analyzed, and clinical data of the study group was compared to that of the control group during
pre-operative and 1- and 6-months post-operative using one-way ANOVA test to compare means.
After comparison of both groups, it revealed that there was significant difference in speech
function between study and control groups at 1-month post-operatively (p= 0.002) while during
preoperative assessment and 6 months post-operative periods there were no significant difference
between both groups regarding the speech (p= 0.643 for preoperative and p= 0.218 for 6 months
postoperative). For radiographic assessment, statistical analysis showed that in the study group
there were no significant difference between preoperative volume and 6 months postoperative
volume p=0.875. Also, there were no significant difference between the preoperative plane volume
and 1-month postoperative p=0.875. While in the control group there were significant difference
between preoperative and 1-month postoperative volume p<0.05 also between preoperative and 6
months postoperative there were significant difference p=0.007.
