Abstract

Twenty one patients presented with femoral neck fracture underwent closed reduction and internal fixation using the Biplane double supporting screw fixation (BDSF) technique. Age group ranged from 18 : 77 years, 17 male and 4 females, 13 cases were fixed within first 24 hours and 8 were fixed after 24 hours. Regarding Garden classification: Type I: 2 cases, II: 1 case, III: 10 cases, IV: 8 cases; 4 cases had posterior comminution. Regarding pauwels classification: Type I: 4 cases, II: 13 cases, III: 4 cases. Eighteen (18) cases were transcervical, 2 subcpital and 1 basicervical. All cases are done under spinal anesthesia, with closed reduction and internal fixation with the BDSF technique. Patients are followed up monthly until union. BDSF is a new method of internal fixation, designed to improve the internal fixation strength of intracapsular femoral neck fractures depending on the original concept of the establishment of two supporting points for the implants and their biplane positioning in the femoral neck and head. The provision of two steady supporting points for the implants and the highly increased (obtuse) angle at which they are positioned, allow the body weight to be transferred successfully from the head fragment onto the diaphysis
