Management of advanced non-small cell lung cancer is rapidly evolving. Advances in combined chemoradiation therapy led to improvements in patient survival, but most patients relapse making it necessary for exploration of new radiation schedules aiming at better local control & survival. Study was performed in Clinical Oncology Dep,Faculty of medicine.Cairo University between January 2010 & January 2012. 50 patients stage III inoperable newly diagnosed Stage IIIA & IIIB NSCLC histologically or cytologically proven (AJCC sixth edition), {u2265} 18 years; {u2264} 70 ,Karnofsky performance (KPS) > 70, No prior treatment, Normal kidney & liver functions, No distant metastases, No severe heart or pulmonary disease. Patients were recruited &randomized after 2 cycles of induction chemotherapy (gemcitabine-carboplatin) into 2 groups; 25 treated by a concomitant boost & 25 by sequential boost using 3D conformal beams to a total dose 64.2 Gy Group(A) received 52.2 Gy /29f/6w with concomitant boost 12 Gy/8f (at least 6 hours between 2 sessions) given during first 2 weeks, Group(B) conventional fractionation with sequential boost over 8 weeks. Primary end point was progression free survival ,secondary end points response rate and toxicity.
