Abstract The present study used to use two techniques for bone osteotomy in the buccal wall of the posterior mandible to expose the inferior alveolar Nerve namely conventional bur with motor and piezoelectric surgical unit. Patients were classified in two equal groups (three patients each including four lateralization procedures as two patients operated unilaterally and one patient operated bilaterally) according to the instrument used in osteotomy creation (either conventional burs or piezo apperatus) control group:osteotomized by conventional bur study group:osteotomized by piezoelectric device. All patients underwent local anaesthetization procedures and mucoperiosteal flap reflection. At planned osteotomy, bone cut was initiated to creat a bony window to expose inferior alveolar neurovascular bundle by using a conventional bur in control group, and a piezotome device in study group, then retract the bundle to insert the implants, the neurovascular bundle was then rested passively against the previously inserted implants.
