Abstract

Acute myocardial infarction (AMI) is a serious health problem that requires special standardized nursing intervention, as well as the need for qualified specialized critical care nurses to manage those patients with such problem, identify and prevent complications. The clinical pathway (CPs) for patients with acute myocardial infarction can be defined as a structured, multidisciplinary care plan that outlines the crucial steps in providing healthcare for a particular patient group within a given timeframe to promote evidence-based practice, reduce practice variation, and ultimately improve the quality of care and outcomes. It includes the initial assessment of the patient, implementation of the treatment plan, assessment of the patient’s response to treatment, and guidance for monitoring and follow-up of the nursing intervention.
The clinical pathways (CPs) purpose is to connect both evidence and practice along with optimizing the medical results while maximizing the clinical efficiency as they link the practicing world with the world of knowledge. Moreover, the CPs can reduce the total emergency treatment time, thus reducing the length of hospital stay, medical costs, myocardial infarction recurrence rate, incidence of complications, and cost effectiveness. All of which help in enhancing the rate of overall responses, improving the indicators of cardiac function, improving the negative mood scores, improving the patient‘s outcomes and quality of their life. In addition, reducing the pressure and workload on the health care providers with increasing the nursing satisfaction. Therefore, the present study objective is determining the effects of implementing a developed CPs on the patients with AMI.
Research Design: The researcher used a quasi-experimental research design for determining the effects of implementing a developed clinical pathway (CP) for patients with acute myocardial infarction (AMI).
Materials & Method:
Setting: the present study was carried out in the Coronary Care Unit (CCU) at Alexandria Main University Hospital.
Subjects: a convenience sample of sixty newly admitted, adult patients with AMI included in the current study. The patients who met the inclusion criteria were equally and randomly assigned into two groups: the intervention group and the control group (thirty patients each).
Study tools: To fulfill the aim of the present study, four tools were used for data collection from the studied patients.
Tool one: Acute myocardial infarction patients’ assessment, all the participants were assessed using this tool that the researcher developed based on a thorough literature review. Tool one was applied for evaluating the patient‘s physiological condition on daily basis from the admission‘s day up till the transfer to the ward or hospital discharging.
It consisted mainly of three parts: the patients’ socio-demographic data; admission clinical data; and ongoing clinical parameters. It was used for assessing the patient‘s vital signs (temperature, pulse, respiration, and blood pressure); any difficulties such as heart failure, arrhythmias, and pulmonary embolism; discharging criteria from CCU, such as.
