Abstract

The present study was conducted to compare two techniques of multiple point TMJ prolotherapy using dextrose versus liquid platelet rich fibrin in management of internal derangement of the temporomandibular joint. Thirty patients were selected complaining from pain, limited jaw movements and joint noises.
Clinical examination was performed for each patient in the beginning of treatment and patients are divided into two groups
group A: 15 patients with internal derangement were treated with injectable dextrose prolotherapy
group B: 15 patients with internal derangement were treated with injectable PRF prolotherapy
Follow up was done at 1 week, 1, 3 months and 6month intervals. Postoperative evaluation was performed and the collected data was statistically analyzed.
Prolotherapy solution was injected under infiltration anesthesia at three target sites which are posterior joint space, anterior disc attachment and masseter attachment.
For patients group B (injected liquid PRF)
Liquid platelet-rich fibrin preparation
Blood was collected from the antecubital vein through an aseptic technique of blood collection, using (sterile uncoated plastic tubes) without additive and immediately centrifuged. The low speed centrifugation protocol used to obtain liquid PRF was (700 rpm) for 3 min.
The parameters which assessed preoperatively and postoperatively after 1 week, 1, 3, and 6 months are pain level intensity, range of maximum mouth opening, range of lateral movements, range of protrusive movements , joint noises and joint tenderness.
Regarding the follow-up parameters pain level was significantly lower in group B (I-PRF) at 1 week, 1 month, 3 months and 6 months intervals. The maximum inter-incisal mouth opening is significantly higher in I-PRF group at 1 month, 3 months and 6 months. A statistically insignificant difference was reported regarding joint tenderness, range in jaw movement, and joint noise.
