Abstract

Eustachian tube is a tube connecting middle ear cavity with nasopharynx and has a very important role in regulation of pressure around TM.
ET function tests are important to evaluate its health and performing its functions. One of these function tests are tympanometry and CT contrast injection.
Myringoplasty means reconstruction of TM in cases of chronic suppurative otitis media with central perforation ” dry and safe ” type, which may have a role or effect on ET function post- operatively.
A prospective study included 30 patients who admitted to E.N.T Department at Sohag University Hospital during the period between December 2020 to December 2022 to undergo myringoplasty.
Analysis of medical charts of the patients was done to investigate variables such as gender, age at the time of surgery, type of hearing loss and its degree, otological examination, pre-operative E.T function tests using tempanometry and CT contrast injection.
Intra operative injection of saline in E.T orifice was done to all cases. Periosteal graft was used in all patients by underlay technique through post auricular incision. Post operative follow up for 3 months was recorded and cases with taken graft re-examined for E.T function by tympanometry to evaluate the effect of myringoplasty on it.
Statistical analysis was done for cases. Results of the study showed that there were 25 cases with taken graft, two patients had patent non-functioning ET preoperatively and turned functioning by tympanometry post-operatively; two patients had obstructed non-functioning ET preoperatively, with also obstructed ET during intraoperative saline injection and turned functioning by tympanometry post-operatively; two patients had obstructed non-functioning ET preoperatively, with one of them had patent and the other had obstructed ET during saline injection, and still non-functioning by tympanometry post-operatively; while nine patients had functioning ET preoperatively by tympanometry with stenosed ET in 2 cases, both revealed intraoperative saline patency and obstructed ET in 7 cases, three of them revealed intraoperative saline patency while four cases showed obstructed ET during saline injection and still functioning by tympanometry post-operatively.
We recommend more studies with large number of patients and studying of mastoid cavity by CT before operation, status of middle ear mucosa and E.T function by other tests than those mentioned in this study.
Conclusion and Recommendation
Eustachian tube has a very important role in health of middle ear. Its patency is not the only key point in the pathogenesis of CSOM but its dysfunction or inflammation increases the risk of the disease
There are many E.T function tests one of these is E.T function test by tympanometry and CT contrast delineation.
Eustachian tube is a mysterious structure that it’s function can never be predicted accurately by any mean. As some of such cases proved to be obstructed objectively on CT delineation, yet, with successful outcome, Others, proved to be patent , yet with poor outcome, the explanation is vague
In our study there was no relationship between patency of ET on CT contrast delineation and success of myringoplasty.
If ET test proved to be obstructed, don’t hesitate to perform middle ear reconstruction as the tube is a mysterious one.
